Welcome to PTxSA. These Terms and Conditions govern your use of our website and services. By accessing or using our website, scheduling systems, intake forms, or services, you agree to these terms. If you do not agree, please do not use our services.

1. Services Provided

PTxSA offers mobile concierge physical therapy services for athletes. Services include movement assessments, manual therapy, rehabilitation, and performance training. All treatment is one on one and by appointment only.

2. Eligibility for Services

You must be at least 18 years old or have parental or guardian consent to receive services.

Services are intended for athletes and individuals seeking specialized orthopedic and sports rehabilitation.

PTxSA does not accept insurance. All services are cash pay or self pay.

3. Appointment Policies

Scheduling:
Appointments may be scheduled through our website, phone, or direct communication.

Cancellations and Rescheduling:
Cancellations within 24 hours of a scheduled appointment may result in a cancellation fee up to the full session cost.
Rescheduling is subject to availability.

No Show Policy:
Failure to appear for a scheduled appointment without notice may result in being charged the full session fee.

4. Payment and Fees

Payment is due at the time of service unless otherwise agreed in writing.

Accepted payment methods include credit or debit cards and approved digital payment services.

No refunds are issued for completed services.

5. Program Name: PTxSA Appointment & Care SMS Program

 

By providing your mobile phone number and opting in through our website, intake forms,

or scheduling systems, you agree to receive SMS messages from PTxSA related to your care. Message types may include appointment reminders, confirmations, rescheduling notices, follow-ups, care instructions, and required intake or paperwork notifications.

 

Message frequency varies. Message and data rates may apply.

Consent to receive SMS messages is not a condition of treatment.

Opt-Out Instructions: You may opt out at any time by replying STOP to any message. After opting out, you will receive a confirmation message and no further SMS messages will be sent unless you opt in again.

PTxSA is not responsible for injuries, complications, or adverse effects resulting from treatment, except in cases of gross negligence or malpractice.

We are not liable for losses, damages, or costs related to missed communications, technical failures, or carrier related SMS delivery issues.

8. Intellectual Property

All content on this website, including text, images, videos, and logos, is the property of PTxSA and may not be copied, reproduced, or distributed without written permission.
